Telehealth Consent
This Telehealth Consent explains important information about using telehealth-enabled intake, clinical review, follow-up, and care coordination workflows connected to Clean Health Rx.
What telehealth means
Telehealth uses electronic communications, secure forms, messaging, audio, video, or related technologies to support medical evaluation, treatment planning, prescription review, follow-up care, and patient education.
Consent to telehealth
By using telehealth-enabled workflows connected to Clean Health Rx, you consent to receiving services through electronic communications and understand that a licensed provider determines whether telehealth is clinically appropriate.
Potential benefits
Telehealth may improve access to care, reduce travel, support timely follow-up, and allow providers to review information remotely when appropriate.
Potential risks
Telehealth has limitations. Technology failures, incomplete information, delays, privacy risks, or the need for in-person examination may affect care. A provider may recommend in-person care when needed.
No emergency care
Clean Health Rx and linked telehealth workflows are not for emergencies. If you believe you are experiencing a medical emergency, call 911 or seek immediate in-person emergency care.
Accurate information
You agree to provide complete and accurate information about your health history, medications, allergies, symptoms, and relevant changes. Inaccurate or incomplete information may affect clinical review and treatment decisions.
Prescriptions
A prescription is not guaranteed. A licensed provider determines whether medication is clinically appropriate after reviewing your information and applicable requirements.
Privacy and records
Telehealth communications and related records may be stored, shared, and used as described in applicable privacy notices, clinical partner policies, and legal requirements.
